Office Policies

Financial Responsibility

 

Initial Evaluation: Typically lasts 90 minutes.

Follow-up Appointments: Typically last 30 minutes.

One-time Consultation: Typically lasts 120 minutes.

Extended phone time between appointments: An additional fee may be incurred 

 

Payment Methods: We accept debit and credit card payments. Please note that we do not accept cash or checks.

 

Prepayment: To secure your appointment, we require an advance payment of half the fee at the time of booking. The remaining balance is due at your appointment time.

 

Scheduling Follow-ups: We require that the next follow-up appointment be made before leaving to ensure continuity of care.

 

 

Cancellation and Rescheduling

 

We understand that schedules can change and flexibility is important:

 

Initial Evaluation: Please provide at least 7 business days’ notice for cancellations to avoid a charge of half the appointment fee.

 

Follow-up Appointments: A minimum of 3 business days’ notice helps us manage our schedule effectively; less notice will incur a charge of half the appointment fee.

 

Same-Day Cancellations: We request 24 hours’ notice to avoid incurring the full appointment fee.

 

Note on Missed Appointments: We value our relationship and understand that emergencies happen. However, repeatedly missing appointments may necessitate a review of your care plan with us.

Medication Refill Policy

 

Planning Ahead: We strive to provide prescriptions that last until your next scheduled appointment. Please check with your pharmacy first to see if you have refills remaining.

 

Refill Requests: Please allow us two business days to process your refill requests. To ensure you do not run out of medication, kindly notify us in advance when you are nearing your last refill.

 

Direct Requests: For your security and privacy, we ask that all prescription requests come directly from you, the patient. We cannot accept requests made on your behalf by third parties.

Pre-Appointment Lab Work Policy

 

To ensure the best possible care, we require that all necessary lab tests be completed and results sent to our office before your scheduled appointment. This allows Dr. Kye to make well-informed decisions regarding your treatment and medication management based on the latest health data.

 

Please Note:

  • If bloodwork is ordered for your next scheduled follow up, please schedule your lab work as soon as possible to give you more options in choosing your ideal lab location and appointment time.

  • If lab results are not received in time, we may need to reschedule your appointment to ensure your care is based on the most accurate and complete information (late cancellation fee applies).

Privacy Practices

 

How We Use Your Information:

  • Your personal and medical information is used exclusively for the purpose of providing you with the highest level of psychiatric care, including diagnosis, treatment, and billing.

  • Your information may be shared with healthcare providers, laboratories, or other medical specialists involved in your care, but only as necessary and with your explicit consent.

 

Your Rights Over Your Information:

  • You have the right to review and obtain a copy of your medical records.

  • You have the right to request amendments to your records if you believe there is an error.

  • You have the right to request restrictions on how we use or disclose your medical information, although we are not always required to agree to these requests.

 

Our Responsibilities:

  • We are required by law to maintain the privacy of your information and provide you with this notice of our legal duties and privacy practices with respect to that information.

  • We will notify you if there is a breach of your unsecured personal health information.

  • We will not use or disclose your health information without your authorization, except in specific instances outlined in our privacy policy or when mandated by law. Such instances include necessary disclosures for public health and safety, compliance with legal proceedings, or cooperation with law enforcement authorities.
